WHEN TO CONTACT A GERIATRIC CARE MANAGER

Family members, health care, legal, accounting and financial professionals may need the services of a Geriatric Care Manager when:

  • Family members live far away from their relative (s).
  • They are unfamiliar with medical conditions and terminology and need assistance coordinating care with medical personnel.
  • Their loved one or client lives alone.
  • An older person's support system is questionable.
  • A recent trauma or sudden change in functioning has occurred.
  • The client is caregiver for a family member, and something happens to the caregiver.
  • Abuse or neglect is suspected.
  • Multiple medical problems need monitoring.
  • Need exists for community resources.
  • Long term planning and placement is required.
  • Non-compliance with the treatment plan occurs.
  • Depression and anxiety exist.
  • Recurrent hospitalizations occur.
  • Terminal illness exists.
  • Substance abuse is suspected.
  • The legal or financial professional needs care coordination by a human services professional. Such coordination could include home health care, companions, daycare, rehabilitation or other medical issues.
  • Personal advocacy for the client or family is needed in the community or nursing home.
